I agree that the club location is Snap Fitness Beecroft, 4 Hannah St Beecroft NSW 2119 Australia.

DEFINED TERMS. In this agreement. "Club Owner" refers to Strathfield Fitness Pty Ltd doing business as SNAP FITNESS. "Club Location" refers to the club at which the agreement is signed. "I" refers to the person whose name and signature appear on this form.

I represent that I am in good physical condition and have no medical reason or impairment that might prevent me from my intended use of Snap Fitness facilities. I acknowledge the Club Owner and Snap Fitness wilt not and cannot provide to me medical advice relating to my physical condition and ability to use the facilities.

If I have any health or medical concerns now or after I join, I will discuss them with my doctor before using the facilities.

If my physical condition or physical limitations, now or in the future, limit my use of the Snap Fitness facilities. I agree that my right to use the facilities will be limited accordingly.

I assume all risks of injury and waive all rights to pursue money damages or any other relief of any kind as a result of anything occurring at or near the Club Location or any other "Snap Fitness'* location.

In the event I am injured while on a Snap Fitness property or during a Snap Fitness-sponsored event, I will hold harmless Club Owner, Snap Fitness Inc. and all of their owners, employees, agents, successors and assigns from all claims of any sort for damages or for other relief, including but not Limited to claims for contrlbution.

I understand and agree that the Club Location and all Snap Fitness Locations are unsupervised fitness centres and no employee is on site to help me use the equipment or exercise in the manner that I choose to exercise.

I acknowledge there is possible danger connected with any physical activity (including the dangers of physical injury and death) and knowingly and voluntarily waive my right to make a Legal or equitable claim of any sort against Club Owner or Snap Fitness, Inc. and all of their owners, employees, agents, successors and assigns from all claims of any sort for damages or for other relief.

This includes but is not limited to claims for contribution. This assumption of risk and waiver of liability applies to my family members, successors, heirs and assigns.

I understand that Club Owner and Snap Fitness, Inc. are not responsible for any of my personal property that is damaged, Lost or stolen while or around the Club Location or any other "Snap Fitness" location.

I am liable for all damage that I cause to equipment or physical infrastructure of the facility and will reimburse Snap Fitness.

I understand i may be expected to show photo I.D. when I enter the facility. This pass and access card are only good for this Club Location. I wilt not let anyone else use my access card and I will not bring any unauthorised guest into the Club Location.
